Providence Health & Services Patient Navigator - Oncology in Anchorage, Alaska
Providence is calling a Patient Navigator (Oncology Support Services, Day, 1 FTE) to Providence Alaska Medical Center in Anchorage, Alaska.
In this position you will:
Provide individualized assistance to cancer patients, families, and/or caregivers to help overcome healthcare system barriers and facilitate timely access to quality health and psychosocial care from pre-diagnosis through all phases of the cancer experience.
Work in collaboration with patient, families, caregivers, providers, Cancer Center colleagues and staff, and other community services to assess, plan, implement, and evaluate appropriate education, care, and/or resources to be provided.
Work with local and statewide cancer care community to provide and assist in education, community needs assessment, resolutions of system barriers, cancer health disparities, understand cultural competence, and the individualized provision of assistance to patients with cancer, their families, caregivers, and survivors at risk.
Required qualifications for this position include:
Master's degree in Social Work (MSW)
Four years of experience in a medical or counseling setting
Ability to communicate well and interact with multiple health care professionals, including medical and allied staff
Preferred qualifications for this position include:
Licensed Master of Social Work (LMSW) or related field; Licensed Clinical Social Worker (LCSW) preferred
Knowledge of Alaska community resources
