Providence Health & Services Home Health Access Coordinator in Portland, Oregon
Providence is calling a Home Health Access Coordinator (0.6 FTE, Days) to Providence Home Services Building in Portland, OR.
Friday, Saturday, Sunday - 8:00 a.m. to 4:30 p.m. Also ability to cover vacations.
In this position, you will:
Screen, prioritize and triage incoming phone calls.
Arrange prescribed services, whether initial or ongoing, with the physician, payor, patient/caregiver, and provider to ensure that services are provided in a timely and coordinated fashion.
Be responsible for monitoring patients who are managed with remote monitoring equipment and triaging program patients.
Perform the duties under direction of the Supervisor Clinical Intake/Scheduling Supervisor.
Required qualifications for this position include:
Graduation from an accredited Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) school of nursing or Physical Therapy Assistant (PTA) program
Current license to practice in the State of Oregon as an LPN/LVN or PTA
2 years of experience as an LPN/LVN or PTA (Acute, Clinic, or Long Term Care)
1 year of Home Health experience
Preferred qualifications for this position include:
- 1 year of triage experience
