Providence Health & Services Admissions Social Services Program Coordinator in Seward, Alaska


Providence is calling an Admissions Social Services Program Coordinator (Inpatient Social Services, Day Shift, 1 FTE) at Providence Seward Medical Center in Seward, AK.

The Admissions Social Services Program Coordinator serves as a liaison between the elder, family, referral source and the interdisciplinary team so that everyone is prepared and informed to facilitate a safe, timely and positive admission experience.

In this position you will:

  • Receive referrals and screens applicants for appropriate placement and eligibility for nursing home services. Gather and assimilate information using a standardized application packet and interview format that will be used by an interdisciplinary team to determine if PSMH is able to meet the needs of the elder. Coordinate admissions tasks to ensure that activities are completed, accurate and timely. Keep applicants/families and referring agencies informed in the process. Serve as a liaison between the applicant/referring agency and the Interdisciplinary team.

  • Facilitate room moves and work with Lodge Teams to anticipate each elder’s needs related to the room selection and arrangement. In conjunction with the interdisciplinary team, determine and ensure delivery of necessary equipment to address each individual resident’s needs so that there is a smooth transition on the day of admission. Communicate a detailed admit memo to the care team 24-72 hours prior to admission.

  • Provide financial counseling and assist residents or their representatives with procedures related to facilitate payment for nursing home services. Communicate payer source information to Finance Department.

  • Verify insurance coverage and communicate that information to appropriate staff and residents or family members. Initiate Medicaid processes before, during and after admission. Verify Medicare Part D coverage for all new admissions. Assist with signing residents up for Medicare Part D coverage, as well as acting as an on-going resource for all eligible Medicare Part D residents. Complete paperwork associated with Medicare Part D Prescription Drug Plans related to resolving formulary and non-coverage issues. Follow-up with new residents that need assistance with coverage.

  • Track and prepare reports related to census, Continuing Care census, bed grid and admission reports.


Required qualifications for this position include:

  • High School Graduate

Preferred qualifications for this position include:

  • Bachelors Degree in Social Work or other health related field such as Psychology or Human Services

  • Current license or certification in Social Work or other health care field

  • 2 years experience in geriatrics and long-term care

  • Prior experience in accounting and financial planning

  • Knowledgeable with State and Federal agencies

  • Knowledgeable of Medicaid and Medicare programs

  • Proficiency with Microsoft Office Suite

  • Prior experience working with Electronic Medical Records (EMR) such as EPIC
