Providence is calling a Social Worker – Ambulatory Care (0.6 FTE, Days) to work in Behavioral Health Urgent Care at Providence Medical Group Northwest Washington in Everett, WA
Under general supervision of the manager of care management, the Social Worker is to work with and support the providers through the provision of resources, advocacy and referrals that address patient's social determinants of health. The Social Worker is also an internal resource for community referrals and linkages. They will partner with the interdisciplinary team (Behavioral Health, PSAR, pharmacists, PCP's, RN Care Managers) to help patients access appropriate care.
In this position you will:
Makes referrals, based on identified needs, to appropriate agencies/services or provides information and assists clients for self-referral. Partners with Care Management, Behavioral Health, and Patient Service Access representatives to help patients receive appropriate care and resources.
Provides advocacy as needed to encourage participation from clients in resolving problems (such as transportation or language barriers).
Functions within the scope of practice for Social Work and Psychosocial Services as detailed by the NASW. Utilizes the NASW Core Clinical Indicators as a standard of care and measurement of needs for education and/or additional training needs.
Assists client in connecting with staff to help with tasks necessary to access services such as filling out appropriate forms, obtaining necessary documentation or authorizations.
Maintains professional standards and follows the treatment policies and procedures of the clinic, as applicable federal, state and local regulations.
Completes a daily schedule which documents every client visit, and time of visit according to clinic policy.
Follows mandatory reporting guidelines for CPS/ APS.
Communicates clearly, both orally and in writing, with clients, providers, community professionals, and interdisciplinary team members. This includes maintaining records that are clear and concise regarding psychosocial and/or needs assessment, advocacy, referrals, and follow-up care. The documentation must reflect legal mandates for reporting abuse/neglect.
Demonstrates knowledge and skills necessary to provide care appropriate to the age and/or diversity of the patients served.
Contributes to the plan of care as established by the inter-disciplinary team and works to obtain the necessary services and resources related to the patient’s needs.
Demonstrates positive and professional interpersonal relations in dealing with others, recognizing cultural diversity, including patients and families, employees, management team, medical staff, volunteers and community members, so that productivity and positive relations are maximized. Maintains appropriate boundaries in all interactions.
Required qualifications for this position include:
Master's Degree in Social Work
Experience in Case management
Social Worker must possess current knowledge of the bio psychosocial aspects of illness, disability and death and be able to apply this knowledge in a professional and understanding manner in direct social work services to patients and their families and in collaboration with all health care professional staff. Social Workers must have the ability to communicate well and interact with multiple health care professionals, including medical and allied staff. Organizational, problem-solving skills, and communication skills, as well as the ability to work independently as a team member.
Some positions may require specific experience depending upon area of hire
Preferred qualification for this position include:
- Experience working in an out-patient setting.
