Providence Social Worker - Cancer Center Partnership in Everett, Washington
Providence is calling a Social Worker (1.0 FTE, Day Shift) to work in Cancer Center Partnership at Providence Regional Medical Center Everett in Everett, WA.
Develop and coordinate patient and family assessments, provides emotional support for patients and their families and assists other members of the treatment team in the integration of psycho-social issues into plan of care management.
In this position you will:
Demonstrates application of knowledge, skills and behaviors in the physical, motor/sensory, cognitive, psychosocial, patient and family education areas based on PRMCE age-specific care.
Documents psychosocial interactions in patient's chart for those patients assigned to her/him based on referrals, dept policies, or patient/family requests.
Enters patient documentation in EMR or other electronic medical record systems as appropriate.
Communicates orally and in writing in the chart with other team members and disciplines so as to insure continuity of care, improved coping, adjustment to diagnosis and treatment – compliance.
Efficiently and appropriately helps patients access resources in the community as needed.
Required qualifications for this position include:
Master’s degree in Social Work
Experience in complex social work interventions in a hospital setting or a medical setting
Licensure as a Social Worker (LICSW or LASW) within 6 months of hire or transfer into position
Preferred qualifications for this position include:
Counseling and Oncology experience
OSW-C Board of Oncology Social Work Certification
