Providence Health & Services Social Worker MSW - Hospice in Everett, Washington
Providence is calling a Social Worker (MSW) (Hospice, Day Shift, 1.0 FTE) to Providence Hospice and Home Care of Snohomish County in Everett, Washington. This position is responsible for providing services to hospice clients, families, and caregivers, addressing a wide range of psychosocial issues related to end-of-life needs including: adjustment to illness/prognosis, care system issues, community resource issues, insurance/financial issues, legal/ethical issues, and end-of-life planning. The MSW develops and implements a plan of care in collaboration with the Hospice Interdisciplinary Team.
In this position you will:
Provide medical social work service to patients and families including assessment, counseling, community resource referral, advocacy, crisis intervention, and discharge planning utilizing current practice knowledge and skill.
Consult and educates staff, volunteers, and community of the social and emotional factors related to the End of Life Care.
Provides social work in accordance with the NASW Code of Ethics and Providence Hospice & Home Care of Snohomish County’s philosophy, policies and procedures.
All required documentation will be complete, accurate and timely.
Promote and practices patient/family, personal and environmental safety while performing job duties according to agency policies and procedures.
Required qualifications for this position include:
Master’s degree in Social Work (MSW) from an accredited school
Current WA Registered Counselor/Social Worker Licensure.
Current Healthcare Provider CPR certification.
Valid Washington State driver’s license and have access to daily use of an insured reliable vehicle or an alternate ability to travel to sites as required.
Proof of current Auto Insurance.
1 year experience in health care setting.
Preferred qualifications for this position include:
Experience working in a hospice care setting
2 years of work experience in a healthcare setting
