The Case Manager/RN Navigator is responsible for the comprehensive assessment, planning, implementation, and evaluation of individual patient needs for discharge planning, admission to swing bed and case management including utilization review in accordance with Providence philosophy, objectives, policies, procedures and standards. The Case Manager/RN Navigator partners with the Utilization Review nurse, patients, physicians, and interdisciplinary team to ensure authorization from third party payors secured; assure resource access and function as team member providing coordinated admission and discharge planning. Fosters change and teamwork to ensure constructive relationships and growth, while maintaining a positive and professional image when interacting with patients, their families, physicians, employees and general public.

Graduate of an accredited/approved school of Nursing.
Associate's Degree in Nursing.
Alaska Registered Nurse License upon hire.
3 years Clinical nursing experience.
Prior experience with provider requirements for admissions transfers and discharges.
Prior experience using Electronic Medical Records (EMR) such as EPIC.
1 year experience as an RN in Utilization Management-review, Case Management and/or Discharge-planning.
Experience with InterQual criteria, Medicare and Medicaid regulations.
Prior experience using Microsoft Office Suite
