Providence Health & Services RN - Hospice Sound in Lacey, Washington


Providence is calling a Registered Nurse (RN) (Per Diem, Variable Shift) to work at Hospice Sound in SCS Hospice in Lacey, WA.

The Registered Nurse involves certified nursing work managing the plan of care for Hospice patients. This position provides direct health care services to Hospice patients and their families in their homes, and must demonstrate independent clinical expertise throughout the nursing process. This position works collaboratively with members of the interdisciplinary hospice team, and oversees the work of appropriate caregivers (such as Hospice Aides and/or LPN) to ensure quality patient care and to enhance the quality of life of dying patients. This position is supervised by a Hospice Clinical Manager, through patient care documentation and electronic medical records, written and verbal communication, and direct observation of patient care. Shift, schedule and caseload may vary depending on position assignment.

In this position you will:

  • Nursing care needs are assessed as needed for a defined group of clients; after a referral, an accurate history and physical exam will be performed; document assessment data within 24 hours of home visit.

  • Determine priorities of client care based on client's needs and available resources; manage time effectively; prioritize client's needs effectively.

  • Provide information to client & significant others regarding diagnosis, interventions & available resources.

  • Maintain open communication to client & significant others.

  • Assess, plan, implement & evaluate educational needs and readiness of client & significant others to learn; develop individualized teaching plan tailored to the client's goals; implement & document teaching plan; provide concise teaching materials & practical technique demonstrations that are for the home setting; modify plan after evaluation process as needed; provide or assist patient and significant others in obtaining equipment & supplies necessary to provide quality care; encourage compliance with therapeutic nursing and medical regimes.


Required qualifications for this position include:

  • Graduate of a school of nursing

  • Two (2) years nursing experience

  • Current Washington State Registered Nurse License

  • Valid Washington State driver’s license, insured and reliable transportation

  • Healthcare Provider BLS/CPR certification

Preferred qualification for this position include:

  • One (1) year hospice, home health, skilled nursing facility, medical/surgical, or oncology
