Providence Clinical Pharmacy Specialist in Medford, Oregon


The Clinical Pharmacy Specialist will collaborate with providers and caregivers to provide coordinated, comprehensive, high-quality care to patients referred to the PH&S Anticoagulation and Pharmacotherapy Services for focused drug therapy management of acute and chronic disorders. The Clinical Pharmacy Specialist is responsible for providing collaborative drug therapy management (CDTM) services in clinic, telephonically, by e-visits, and possibly telehealth. Upon referral, the Clinical Pharmacy Specialist will assess the patient, initiate and adjust medications, screen for and minimize drug interactions and adverse effects, provide disease/medication education, and explore opportunities to simplify &/or decrease the cost of the medication regimen in collaboration with the referring provider (PCP)/prescriber, patient/their caregiver and other Providence team members. They will also educate team members to continually advance the service level of practice.

$1,000 hiring bonus for eligible external hires that meet required qualifications and conditions for payment.

Required Qualifications:

  • Graduate from an accredited school of pharmacy.

  • Coursework/Training: Post-graduate year residency (PGY1) or 3 years experience in area of specialization (ambulatory care with direct, collaborative patient care) in lieu of residency.

  • Oregon Pharmacist License upon hire

  • National Certification from Board of Pharmacy Specialties within 2 years of hire

  • Hands-on experience in relevant patient assessment, medication adjustment, monitoring, and all other aspects of patient care

  • Experience in writing, including educational pieces for patients and healthcare providers.

  • Able to be licensed as a preceptor in the state of Oregon.

  • Experience precepting students and/or residents.

Preferred Qualifications:

  • Completion of a PGY1 or 2 in Ambulatory or Acute Care

  • BPS Board Certified
