Providence RN Manager, Care Management in Medford, Oregon
Providence is calling a RN Manager for its Case Manager team at Providence Medford Medical Center in Medford , OR.
We are seeking a RN Manager for our Case Management team that will be accountable for the direction and management of the Providence Medford Medical Center Care Management Department. This position provides leadership and oversight for the clinical care management model, team staff members, a large departmental budget and general operations of the department.
They will promote team functioning, advance the clinical care management model and improve coordination of patient care in collaboration with all other disciplines. Responsible for adherence to budget and resource targets.
Work with Regional and system CM leaders, Providence Health Plan Manager of QMM Disease and Case Management, Providence Health and Services Oregon leaders and clinical professionals, health plans and Coordinated Care Organizations, Clinic Leadership, Providence Medical Group providers, and community and government agencies as appropriate to establish effective discharge planning mechanisms and develop new models for transitioning patients back into home and community settings.
The CM Manager will effectively interact with Medical Staff, facility executives/leaders, Utilization Management, Finance and continuum and community partners. At the facility level, the CM Manager coordinates and provides oversight of use of the Patient Assistance Fund programs. This position will operate as a strategic partner to other Oregon PH&S facilities in meeting the Triple Aim.
In this position you will have the following responsibilities:
Responsible for Care Management Department activities 24/7
Is subject matter expert and provides leadership in Care Management to Care Management staff as well as interdisciplinary partners
Assure that Care Management staff are competent in their roles, particularly in the clinical care management model, works with staff to develop and maintain skills
Actively collaborate with other PHSOR CM leaders to advance the Clinical Care Management model and leverage our collective resources and talents across PHSOR CM departments and functions
Provide input and support to the regionalization of the Care Management model across ORPHS
Manage day to day operations of the department which includes but is not limited to:
Providing feedback on employee performance
Hiring of all Care Management staff
Performance coaching and evaluations
Managing administrative operations (budget creation and management, job descriptions/roles, policies/procedures, scheduling and assignments)
Orientation and ongoing education of staff
Track and analyze financial, operational and performance data, including the preparation of summary reports and targeted actions as needed
Develop and maintain collaborative relationships with department customers and collaborative partners in providing services
Is responsible for continuing personal and professional growth by maintaining knowledge base in area of specialty. Develop expertise through ongoing education and experiences
Required qualifications for this position include:
Bachelor's Degree in Nursing
Current, unencumbered Oregon RN license
5 years progressive supervisory/management experience including program planning, performance management, staff development and evaluation
3 years Care Management experience
Preferred qualifications for this position include:
Master's Degree in Nursing
2 years relevant social services utilization management and/or quality management
