We are seeking a Manager PACE Program - Milwaukie Admin that will provide overall leadership and management for the day to day operations at the ElderPlace health and social center. Responsible for the general milieu of the site including the day center, medical care and services, clinic operations, dining experience, food service operations, reception and administrative support to the interdisciplinary team and functions. This positions also assures that the clinic has administrative support for processing orders and supply management. The PACE Operations Manager is responsible for daily facility operations, including staff oversight, development/implementation/maintenance of workflows & PACE regulations and onsite trouble shooting. The PACE Operations Manager is also responsible for community outreach, representation and relationships in the site geographical area, as well as with all contracted community providers and partners.
Effectively lead the interdisciplinary team in geriatric participant care planning, problem solving, and conflict resolution.
Manage a complex, multi-discipline program within allocated resources.
Manage staffing pattern and shared staff to assure quality care is provided to geriatric participants.
Monitoring competencies of assigned staff to assure the highest quality of care possible.
Practice and demand service excellence for all customers.
In this position you will have the following responsibilities:
Provide day to day management and oversight for the Elderplace health and social center operations, including all services for center participants, and efficient use of resources.
Supervise assigned ElderPlace staff, including hiring, performance appraisals and disciplinary actions. Assist in providing performance feedback to other Managers for shared or matrixed staff.
Accountability for site census and growth, assuring delivery of targets.
Assure oversight and communication with community housing partners routinely.
Oversee and assure support and contracted services on site at health and social center including supply management, housekeeping, laundry, food provision, etc.
Assure and monitor competencies for assigned staff assuring appropriate training and in-service to maintain or improve competency.
Participate in the department budget process to assure expenses are within targeted levels.
Provide input to Director of PACE Services monthly and as needed.
Liaison with PHS and other contractors, and maintenance reps for repairs and service.
Develop and facilitate effective interdisciplinary team to assure quality participant oversight and care plans.
Provide effective leadership that contributes to staff development and satisfaction.
Implement policies, procedures and standards for all ElderPlace programs and services operating within the site.
Investigate grievances, propose resolutions, and assure timely completion of grievance and appeal processes.
Oversee completion of audits including Ops Manager checklist, hand hygiene, temperature logs, kitchen checklists, coordinating Cooperative Health checks and safety checklists.
Assure compliance with CMS PACE regulations.
Required qualifications for this position includes:
Bachelor's Degree in Nursing, Social Work, Therapy, or related field
In compliance with company policy and state regulations, a Department of Human Services (DHS) criminal background check may be required for this position based upon the location of the position
Preferred qualifications for this position include:
Experience in managing a complex budget
Experience providing care in a managed care environment
