Providence Health & Services Teaching Kitchen Patient Navigator in Milwaukie, Oregon
Providence is calling a Teaching Kitchen Patient Navigator (0.0 FTE, Days) to Providence Milwaukie Hospital.
In this position, you will:
Support nutrition services for the Providence Milwaukie Hospital (PMH) Healthy Teaching Kitchen to help achieve the best possible quality, experience and affordability outcomes for patients.
Provide supervision and support for the volunteer staff and works as part of an integrated care team that includes registered dietitians, onsite volunteers, and clinic staff to effectively address the needs of patients in a collaborative, patient-centered environment.
Have a solid understanding of the various Providence-provided and community-provided resources that relate specifically to food insecurity and can advise patients on how to best take advantage of these resources.
Help oversee the daily operations of the Teaching Kitchen and will ensure the highest level of customer service to patients, families, and community partners.
Required qualifications for this position include:
High school diploma
Valid County Health Department Food Handler's Certification or ServSafe Certification at the time of hire
3-5 years of customer-service experience
Computer skills and the ability to accurately and thoroughly input data
Preferred qualifications for this position include:
Education in a health-related field
3-5 years of progressive job responsibility or supervisory experience
Experience with patient contact
Experience facilitating group sessions and/or discussions
English/Spanish bilingual fluency
