Providence is calling a LVN Discharge Coordinator, After Hours (Full-Time/Evening Shift) to Providence Facey Medical Foundation in Mission Hills, CA.

We are seeking a LVN Discharge Coordinator, After Hours who will be responsible for utilization review, discharge planning and repatriation coordination to in-network facilities for patients on an on-call basis between the hours of 5:00 pm and 8:00 am for Providence Health & Services Medical Foundations. The position requires the coordination of all services related to the transfer of hospitalized members who require transfers to in-network and or higher level of care facilities, or transitions to lower level care i.e. Home, Skilled Nursing/Acute Rehab facilities etc. Responsible for managing safe and medically appropriate care transitions by ensuring that quality standards, including discharge arrangements and repatriation coordination processes, are met.

In this position you will be responsible for:

  • Deliver upon the service expectations of both our patients and fellow staff members by listening to their needs; engaging in positive interactions; and following through on promises made in a thoughtful, efficient, timely and courteous manner so that their total outcome is better than expected.

  • Respect the dignity, confidentiality and privacy of patients.

  • Work in a safe manner, adhering to general safety precautions and standards. Reports any unsafe conditions to their supervisor and/or the safety hotline.

  • Perform utilization review, discharge planning and repatriation coordination on an on-call basis between the hours of 5:00 pm and 8:00 am for Providence Medical Foundations.

  • Work collaboratively with all clinical and non-clinical team members to solve complex discharge issues.

  • Arrange placement of patients in facilities that are appropriate for their level of care requirements.

  • Authorize medically appropriate facility admissions and transportation. Durable medical equipment and home health care per the patients’ health plan benefits are collaboratively organized with the discharge planners.

  • Identify and refer patients to the organizations social work department who might qualify for the California Children Services as appropriate for additional coverage and services.

  • Coordinate transfer of “out of area” patients into in-network facilities.

  • Must be available on an on-call basis to rapidly respond to discharge planning needs as directed by the medical director/hospitalist/case managers/supervisors.

  • Maintain confidentiality of all patient and Utilization Management information.

  • Refer appropriate cases to Social Services and or Behavioral Services for psycho-social intervention.

  • Maintain accurate and thorough documentation of discharge planning/case management activities on the patient’s medical record as well as Utilization Management tracking log.

  • Work collaboratively with other members of the health care team as well as the respective medical groups to facilitate the Utilization Management process.

  • Identify and refer situations needing immediate intervention to the Director of Case Management/supervisor.

  • Participate in UM Department and Quality Assurance programs and projects as needed.

  • Maintain documentation in the patient EHR as needed and as guided by policy.

  • Follow-up on discharged patients by telephone as necessary to ensure delivery of medical equipment, home health visits and assesses health status.

  • Must comply with the requirements and expectations of the Remote Utilization Management & Case Management Telecommuting Policy and Agreement at all times.

  • Must respond telephonically to requests for discharge assistance within 15 minutes of request.

  • Maintain a high degree of professionalism.

  • Demonstrate the ability to make decisions, take appropriate action and follow tasks through to completion.

  • Recognize and analyze the implications of new situations and develops workable solutions to maintain productivity and morale.

  • Act as a role model in demonstrating the customer service standards of the organization.

  • Demonstrate a commitment to personal growth and development by participating in external activities related to professional goals.

  • Respond cooperatively to managers and staff members in other departments to promote teamwork.

  • Attend departmental meetings as appropriate and contributes ideas for improving efficiency, productivity and patient satisfaction.

  • Maintain current appropriate licensure/certifications.

Qualifications:

Required qualifications for this position include:

  • Graduate of an accredited LVN school of nursing.

  • Current California LVN licensure

  • Two (2) years current acute med/surg experience.

  • Experience in transitioning hospitalized patients to lower levels of care such as post-acute setting.

  • Experience in repatriating patients to in-network facilities and or higher level of care transfers.

  • Will perform duties under the direction of the medical director, hospitalist or MSO management.

  • Demonstrates proper phone etiquette, strong computer skills including knowledge of Word and Excel.

  • Must possess excellent communication skills, must be organized, proactive and be able to prioritize work.

  • Position requires strong problem solving skills and the ability to work independently and remotely.

  • Must have knowledge of health plan requirements and benefits as applied to the utilization and case management process.

  • Must respond telephonically to requests for discharge assistance within 15 minutes of request.

  • Must comply with the requirements and expectations of the Remote Utilization Management & Case Management Telecommuting Policy and Agreement at all times.

Preferred qualifications for this position include:

  • Hospital inpatient case management or ambulatory case management experience.
