Providence is calling a Manager Clinical Pharmacy Operations (Full-time/Day-shift) to Providence Facey Medical Foundation in Mission Hills, CA .

We are seeking a Manager Clinical Pharmacy Operations - Provides expertise and vision to Facey leadership in regards to clinical and fiscal pharmacotherapy issues. Works collaboratively with key organization and medical group stakeholders to develop pharmacy strategy, policies, protocols and programs that improve patient outcomes through appropriate medication and disease management. Partners with department director to develop pharmacy reports including but not limited to process metrics, patient satisfaction, clinical outcomes, quality metrics and financial benefits of clinical pharmacy interventions to Facey leadership as needed to drive quality improvement and justify expansion. Collaborates with regional Pharmacy leadership on pharmacy quality initiatives and population health programs. Is active in organization wide ambulatory care pharmacy committees to support growth and expansion of pharmacy services. Supports clinicians and other health care providers through collaborative drug therapy management services, medication conversion programs and other functions to ensure that the use of pharmaceutical agents is rational and cost-effective. Leads Facey's Pharmacy and Therapeutics Committee (P&T) and participates in committees and workgroups relevant to the functions of pharmacy services. Partners with pharmacy refill leadership to optimize integration of all pharmacy services. Supervises, motivates and mentors department ambulatory care pharmacists and department supervisor (CPhT). Hires and retains qualified staff; ensures new caregivers are well oriented to their department. Motivates and manages caregivers to ensure that performance standards are met or exceeded. In this position you will:

  • Administrative/Leadership/Professionalism: Demonstrates effective management skills:

  • Supervise pharmacy providers and department core leaders, performing the full range of supervisory functions, including: performance evaluations; staffing plans; training and development; coaching and mentoring assignment, review and approval of work; leave approvals; disciplinary matters; team building; recruitment

  • Hires and retains qualified staff; ensures new employees are oriented to their department and well trained for their jobs

  • Motivate and manages staff to ensure that performance standards are met or exceeded. Ensures competency is maintained through continuous monitoring, assessment and follow up

  • In collaboration with Director, organizes the department workload, establishes work standards to promote quality, clinical excellence efficiency and productivity

  • Assist the Director with development and implementation of strategic initiatives and goals

  • Demonstrate the ability to make independent decisions, take appropriate action and follow tasks through to completion. Takes accountability for results and works collaboratively with others to address deficiencies

  • Identifies operational deficiencies, gathers relevant data and formulates a plan to address the deficiencies

  • Keep caregivers apprised of organization program, policy and operations updates

  • Use, protect, and disclose Facey’s patients protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards

  • Direct Patient Care:

  • Responsible for providing collaborative drug therapy management (CDTM) in designated Facey services in-person, telephonically, and by e-visits

  • Perform pharmaceutical assessment and reassessment of patient medication regimen including reconciliation and adherence to patients in the ambulatory setting

  • Act as a consultative resource to physicians and other members of the health care team to assess patient’s medications for interactions, duplications, contraindications, potential formulary coverage issues, and cost-effective alternatives, and makes recommendations to the prescribing provider

  • Analyze lab results and other clinical data and provides appropriate clinical monitoring. Orders appropriate laboratory tests necessary for monitoring outcomes of medication therapy according to established protocols or in consultation with the primary care physician

  • Assist in the teaching of health care providers, patients and their family members/caregivers about the patient’s medications, directions for use, and other information as deemed necessary. Identifies barriers to patient compliance and assists patient in overcoming barriers

  • Population-Health/Program Development:

  • Participate in the development of treatment algorithms, drug utilization management strategies, intervention techniques and policies needed to deliver and optimize pharmacy program

  • Participate in the quality assurance and improvement process of clinical pharmacy disease state management and education clinics

  • Proactively identifies performance improvement opportunities through data analysis and discussion with subject matter experts to achieve pharmaceutical-related total cost of care and quality results and justify new and innovative advanced practice pharmacist services and models of care

  • Coordinate with clinical departments, clinical operations, health plans, and relevant stakeholders to drive improvement around identified pharmaceutical-related cost reduction and quality improvement areas and delivers and tracks needed results

  • Develop and implements prescribing protocols in collaboration with medical group leadership

  • Implement and monitors drug conversion programs

  • Stay current with updates to scope of practice and licensure through continuous education. Makes recommendations for updates to regulatory compliance and standards of practice

May label, transport and store hazardous waste as described in annual training

Required qualifications for this position includes:

  • PharmD Doctorate of Pharmacy from a college or school of pharmacy accredited by ACPE

  • Current valid California Pharmacist license

  • Current American Heart Association BLS for Healthcare Providers

  • ASHP-Accredited PGY-1 Residency (General Practice, Ambulatory Care or Community Pharmacy)

  • 5 years advanced practice pharmacy experience disease state management with prescriptive authority and medication use evaluations

  • 3 years pharmacy program development, supervision and management responsibilities

  • Ability to communicate well verbally and in writing

  • Knowledge of all Microsoft software

  • Ability to recruit, manage and supervise advanced practice pharmacists and support staff

  • Experience with electronic medical records and electronic prescribing

  • Knowledge and expertise in clinical pharmacology and disease state management using most up-to-date evidence-based practice

  • Ability to educate and persuade patients and/or family members to alter behavior in accordance with program goals and objectives

  • Excellent organizational, interpersonal and decision-making skills

  • Ability to work independently and manage multiple, concurrent projects for completion in a thorough and timely manner

  • Ability to analyze data and formulate sound conclusions and recommendations

  • General knowledge and understanding of pharmacoeconomics

  • General knowledge and understanding of Accountable Care Organizations, MediCare Part D, HEDIS, CMS Stars, and Value-Based Purchasing

Preferred qualification for this position includes:

  • ASHP-Accredited PGY-2 Residency (Ambulatory Care, Pharmacoeconomics, Managed Care)

  • Certification in Pharmacotherapy, Ambulatory Care, Diabetes or Geriatrics
