Providence Social Worker LCSW in Missoula, Montana

Description:

Providence is calling a Social Worker LCSW (Mental Health, Variable Shift, 0.0 FTE) to Providence Medical Group in Missoula, Montana. In this position you will:

  • Conducts clinical interviews to assess pertinent patient information. Assesses patient/family/caregiver learning needs, abilities, preferences and readiness to learn. Completes on going assessments and documents according to policy. Communicates with other members of the health care team regarding the patient’s needs, plan and response to care. Evaluates response to plan and makes adjustments as indicated.

  • Observe patient behavior accurately; familiarity with specific objective tests to augment and support the findings of clinical interviews.

  • For the ages listed, screens, assesses, monitors, documents and interprets psychosocial, spiritual, environmental, self-care, educational, and patient-centered care planning needs.

  • In collaboration with the patient/family, and in a developmentally and culturally appropriate manner, implements the plan of care according to policy.

  • Assess the patient’s coping skill repertoire.

  • Clearly and concisely document all findings in a standard format.

  • Conduct a comprehensive social history including but not limited to current family situation, family history, vocational and educational history, legal history, trauma history and psychiatric history.

  • Demonstrates the ability to conduct a Mental Status Exam.

  • Demonstrates the ability to function effectively as a member of an interdisciplinary care team.

  • Demonstrates the ability to obtain information relevant to the patients presenting complaint, including but not limited to duration, triggers, important environmental factors, and symptoms of concern. Also assess and address barriers to treatment goals and care plan.

  • Participates in the development of patient plan of care to include patient problems, goals, evaluations and interventions.

  • Reports symptoms and significant changes in patient’s condition to appropriate personnel.

  • Consults regarding cases that are presenting problems and/or experiencing significant deviations from the plan of care, conferring with the Medical Director/Department Director as needed.

  • Utilizes appropriate teaching methods and/or referrals to ensure patient/family educational needs are met; provides community resource information to patient/family and evaluates the effectiveness of teaching, making adjustments as necessary; documents education process on appropriate screen/format.

  • Works with health care team and outside resources to facilitate optimum patient care delivery.

  • In collaboration with family and other team members, systematically and continually assesses the patient’s behavioral health.

  • Maintains current knowledge of reimbursement and regulatory issues of the payers most frequently seen with the patient population.

  • Provide patient information, educational services, and individually targeted behavioral interventions.

  • Communicates method/intervention progress and responses to appropriate members of the healthcare team.

  • Documents critical incidents when necessary. Provides clear, concise and pertinent directions to care team.

  • Provides community networking and education, as required.

  • Educates health team colleagues about Social Work; including the role and unique needs of the case-managed patient populations and serves as a resource for physicians and other departments.

  • Completes patient’s medical records within 10 days of visit. Assures that inter/intra departmental documentation is recorded, distributed and filed accurately and timely.

  • Documents goals, objectives, methods/interventions and family response.

Qualifications:

Required qualifications for this position include:

  • Masters degree in Social Work (MSW)

  • Current Social Work licensure in Montana State, or license eligible in Montana State

  • 3 years of experience with adult and/or pediatric and adolescent populations and familiarity with a broad range of psychiatric disorders
