We are seeking a Speech Therapist - Home Health Yamhill Service Area who will manage and provide skilled Speech/Language Pathology to patients/clients with a wide variety of diagnoses and disabilities, in a home setting in accordance with a physician's plan of treatment, taking into account individual patient's special or age related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client speech/language/swallowing independence in accordance with the individual's functional level and potential.
In this position you will have the following responsibilities:
At the physician's direction, evaluate the patient's/client's current communication/ swallowing/cognitive status and, with other department personnel, participate in developing the plan of treatment and its revision as necessary
Incorporate patient and family as appropriate in goal setting
Develop and implement effective treatments, monitor patient progress, modify and progress patient/client as appropriate
Teach/instruct patients/clients, family/significant others to assist patient/client in achieving treatment goals
Coordinate care with physicians and other professionals regarding treatment progress, expected results and discharge planning
Observe, record, and report changes in the patient's condition or reaction to treatment to the physician
Provide written and verbal instruction on discharge from Home Health, and arrange for outpatient follow-up as needed
Demonstrate flexibility and adaptability to adjust daily schedule to meet service area demands and complete workload
Perform duties in compliance with departmental policies and procedures
Maintain patient clinical records to meet state, federal and other regulatory body standards
Manage a caseload of patients
Maintain a high standard of professional accountability with respect to appearance, demeanor, safety, ethical and legal issues
Establish annual individual goals for own professional development
Participate in the Continuous Quality Assurance process as defined in the department CQI plan
Provide quality care in an efficient, cost-effective and caring manner
Attend staff/team meetings and departmental continuing education in-services
Required qualifications for this position includes:
Master's Degree from an accredited Speech Language Pathology program -OR- equivalent education/experience
Current license in Speech/Language Pathology granted by the Oregon Health Division
Current Certificate of Clinical Competence from the American Speech-Language-Hearing Association and Speech/Language Pathology licensure through the State of Oregon, or eligibility for both
Valid driver's license
Current BLS (Basic Life Support for Health Care Provider) or ACLS (Advanced Cardiac Life Support) required within 30 days of hire
Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire
One (1) year of Clinical experience as a licensed Speech Language Pathologist
