We are seeking a compassionate and caring Care Management Social Worker (MSW) to coordinate the care for complex patients from pre-admission to immediate post-discharge. In a non caregiver capacity, the MSW will facilitate coordination and communication between all members of the health care team in the decision making process to minimize fragmentation of the health care delivery system.
In this position you will:
- • • Assesses patients for care management services after a referral has been made based on the Nursing Admission screening for potential patient issues. • Appropriately identifies actual and potential care issues within scope of practice. • Develops and manages a coordinated plan of care to meet care managed patient and/or family needs within scope of practice. • Recognizes circumstances requiring nursing intervention. • Monitors the patient/family progress toward goal achievement of optimizing functional status. • Organizes and links appropriate services throughout the hospital and community to deliver coordinated care to patient and family. • Evaluates post-acute care environment for accessibility and adaptive needs. • Plans, leads, participate in or follow through with multidisciplinary team meetings as needed for care coordination. • Assesses the teaching needs of the patient and family members based on patient goals and objectives. • Provides coordination to support the patient and family near the end of life or major life changes. • Develops implements, and monitors effective discharge plan in collaboration with the patient, family, physicians, and primary caregivers. May include procuring durable medical equipment, visiting nurse services, extended care facility bed placement, support group referrals, and other services. • Assist director with Identifying a need for new resources if gaps exist in the service continuum. • Collects, analyzes, and uses care management data to continuously improve the quality of care. • Coordinate health education to patients, families, and staff within specific care managed populations. • Monitors and uses financial resources effectively. • Monitors individual cases in relation to established criteria or policy and intervenes as indicated to facilitate progression of care. • Demonstrates appropriate skills for performing this professional role. • Adheres to standards of practice within the department, recognizing learning needs to maximize level of knowledge and expertise. • Performs and documents a comprehensive psychosocial assessment sensitive to the cultural and age-appropriate needs of the patient and family. • Provides recommendations for treatment and/or referral. • Serves as a consultant to the multidisciplinary care team for psychosocial issues. • Provides short term counseling and crisis intervention for patients and families. • Provides and documents psychosocial and mental status assessments with recommendations for treatment and/or referral. • Makes recommendations to the RN care coordinators, staff nurses and other care team members, patients and families for discharge planning and referral. • Provides education and resources to the multidisciplinary care team regarding the psychosocial aspects of patient care and treatment through formal presentation and consultation as well as through informal team interaction. • Supports and contributes to the maintenance of positive interagency collaboration and networking via meetings, phone calls, correspondence and inservice presentation. • Coordinates and leads care conferences of multidisciplinary team and documents actions. • Contributes to the development and implementation of the psychosocial component of care plans and pathways for identified patients. • Actively participates as a member of the multidisciplinary care team. • Performs other duties as assigned
Required qualifications for this position include:
Masters Degree Social Work or Related field
Three (3) years of Social Servces experience with a health care related organization; experience should include a minimum of 2 years in acute care/care coordination.Management and/or Discharge Planning
