Providence Health & Services Social Worker - Social Services in Olympia, Washington


Providence is calling a Social Worker (Social Services, Variable Shift, 0.0 FTE) to Providence Mother Joseph Care Center in Olympia, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.

In this position you will:

  • Conducts Admissions/Discharge processing of residents, as well as crisis intervention, including assessment of client and family needs and relationships.

  • Provides consultation and advocacy on behalf of clients, families, and the facility/agency staff.

  • Provides counseling on such social service topics as adjusting to illness and coping with grief.

  • Completes psychosocial client assessments by utilizing the medical record and interviewing clients (as well as families and others in their “support systems”).

  • Addresses issues such as advanced directives and Power of Attorney.

  • Communicates with the Care Team to aid in the development of plans of care, complete Minimum Data Set (MDS), and recommend interventions to address clients’ psychosocial needs and advocate for clients’ rights and wishes.

  • Maintains facility/agency client rights program; provide in-services to staff on client rights and related social service topics.


Required qualifications for this position include:

  • Bachelor's Degree in Social Work or a related field

  • Two (2) years experience in geriatric and/or medical social work, or a related field.

  • Ability to communicate in English effectively both verbally and in writing.

Our Transitional Care Unit is designed for patients coming out of the hospital who need skilled nursing care, end of life care, or rehabilitation for those recovering from surgery or who have a functional loss from illness, disease, or an accident. Our Long Term Care unit provides skilled nursing in a residential setting, addressing the needs of our residents through creating a sense of community and home in “neighborhoods” and providing a model of care that is resident-centered.
