Providence Health & Services Clinical Pharmacy Specialist-Elderplace in Portland, Oregon


Providence is calling a Clinical Pharmacy Specialist (0.0 FTE, Days) Providence Elderplace facilities throughout the Portland Metro area.

In this position, you will:

  • Work with interdisciplinary teams at Providence ElderPlace to provide pharmaceutical care services to geriatric participants.

  • Provide drug therapy management and consultative services which will include a review of participant’s drug profiles, working with prescribers to optimize drug regimen, monitoring quality of pharmacy practice, and providing inservice education to Interdisciplinary Teams.

  • Serve as ElderPlace’s liaison to the dispensing pharmacy.

  • Focus on optimizing drug therapy outcomes, improving drug utilization, improving patient compliance, preventing adverse drug events, and acting as a drug therapy resource to physicians and other members of the healthcare team.


Required q ualifications for this position include:

  • Graduate from an accredited college of pharmacy

  • Doctor of pharmacy training

  • Current valid pharmacy license by the Oregon Board of Pharmacy

  • In compliance with company policy and state regulations, a Department of Human Services (DHS) criminal background check may be required for this position based upon the location of the position. Fingerprinting may be required also.

Preferred qualifications for this position include:

  • Specialty residency in geriatric care or previous experience in geriatric care drug therapy management
