Providence is calling a Social Worker MSW for a Full-Time/Day Shift at Saint John's Health Center in Santa Monica, CA

We are seeking a Social Worker MSW who will provide assessment, counseling and information and referral to hospitalized patients, outpatients and their families, under the general supervision of the Director, Social Services. The Social Worker will work cooperatively with medical and paramedical staff.
In this position you will:
Perform assessments, including psychosocial, and data collection
Formulate goal directed plan of care based on patient outcomes: Initiate, individualize and modify Standard of Care; Collaborate with other services
Implement care based on department and hospital standards: Follow established protocols and procedures
Evaluate effectiveness of care
Document patient care records as appropriate
Identify patient and family learning needs and demonstrate measures to meet these needs
Delegate to appropriate patient care team members
Apply knowledge of care with selected patient populations appropriates to the age of patients served
Apply skills and demonstrates competency in selected skills including, but not limited to: Perform crisis intervention; Counsel individual patients and their families; Provide patient and family education; Act as a resources for patients and families; Consult with medical, nursing and ancillary staff regarding psychotherapeutic issues of patients
Demonstrate responsibility and accountability for own professional practice in regard to: Attendance, punctuality and patient confidentiality; Positive public relation image and working relationships; Obtaining necessary knowledge and skill needed to maintain competence and certifications; Completing all mandatory requirements, including annual safety programs, CPR, infection control and annual physical
Comply with hospital, department and unit standards, including but not limited to safety, infection control, quality management, environmental, confidentiality, and patient care
Participate in the development and attainment of team, department and hospital goals including: Attending and participating in division meetings; Volunteering or accepting assignments for projects; Mission, vision and team ground rules
Required qualifications for this position include:
Master's Degree in Social Work from an accredited School of Social Work
Ability to perform rapid, accurate psychosocial assessments.
Knowledge of mandatory reporting requirements.
Knowledge of crisis intervention techniques.
Knowledge of issues common to hospitalized patients and their families.
Ability to deal effectively with many people across a broad range of cultural backgrounds and ages.
Ability to work independently with little or no supervision.
Proficient in P.C. software applications including, Microsoft Word, Excel, PowerPoint, Access, Outlook, etc.
Ability to prioritize, organize and coordinate work.
Ability to work effectively with many people at various levels of the organization.
Excellent oral and written communication skills.
Ability to handle multiple tasks and appropriately prioritize.
Ability to clearly communicate with internal and external customers, including, but not limited to, patients, co-workers, management, physicians, and visitors.
Ability to maintain patient confidentiality and privacy in accordance to hospital policy.
Preferred qualifications for this position include:
One (1) year Post M.S.W. work
Psychiatric or medical (including field work pre-M.S.W.)
