Providence LPN - Pace MLK in Seattle, Washington

Description:

Providence is calling a Licensed Practical Nurse (LPN) (Pace MLK, Day Shift, 1.0 FTE) to Providence ElderPlace in Seattle, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.

Manages all medication orders and delivery coordinating with physician, RN, transportation and home care. Participates in direct nursing care in a manner, which assures participants satisfaction and retention.

Offering $5,000 Sign on bonus for external hires! Schedule is Monday - Friday from 8am to 4:30pm! This position will support the Adult Day Health Program at one of our clinic sites in Seattle off of Martin Luther King Jr. Way.

In this position you will:

  • Assist nursing staff in planning and compliance with medication regime for each participant

  • Participates in the admission, transfer, and discharge processes. Administer care based on the nursing process and the interdisciplinary care plan

  • Set up daily medications as prescribed and administers all medications as indicated and documents on the Medication Administration Record form on a timely basis

  • Receives and reviews all medication orders, maintains an up to date MAR, notifies pharmacy and coordinates distribution of all medications

  • Meets all administrative requirements including maintaining legal clinical records, charting, and attending multidisciplinary team care conferences and staff meetings

Qualifications:

Required qualifications for this position include:

  • Graduate of an accredited Licensed Practical Nurse program

  • Current Washington State License as a Practical Nurse

  • Current Cardiopulmonary Resuscitation (CPR) certification

  • Long Term care or related geriatric experience working with the frail and elderly
