Providence is calling a Certified Clinical Care Coordinator - Providence Family Medicine Sherwood (0.0 FTE, Days) to Sherwood Family Medicine in Sherwood, OR. Anticipated schedule is Monday-Friday 8:00am-5:00pm
We are seeking a Certified Clinical Care Coordinator - Providence Family Medicine Sherwood who will be the key liaison between the patient and their Patient-Centered Medical Home team. Team includes providers, clinic staff, and other health care professionals. Clinical Care Coordinator works to ensure safety, best practices, and high quality standards are maintained for the patient across the health care continuum. We anticipate that our CCC will largely support with desktop needs of the team (75%) and from time to time support with direct patient care needs (25%). Apply Today! In this position you will have the following responsibilities:
Provide compassionate care and excellent customer service while managing the workflow between patients and providers
Manage clinical communications between patients, clinic staff, providers, and internal/external customers to optimize flow and coordination of patient care
Facilitate getting immediate resolution for urgent or emergent patient issues; delegate tasks among team members to ensure the best patient outcomes
Collaborate with Providers to manage, process, and resolve complex or sensitive patient issues
Navigate data systems to research and/or record pertinent information
Required qualifications in this position includes:
Completion of an accredited Medical Assistant program
High School Diploma -OR- GED -OR- equivalent educational experience
Certified through a nationally recognized Medical Assistant program such as AAMA, AMT, (RMA) or NCCT
Must have current AHA-BLS (Basic Life Support) for the Healthcare Provider
If position is in Washington State, applicant must meet requirements for certification according to RCW 18.135, State of Washington, and Department of Health
Candidates are needed to obtain appropriate certification(s) within designated timeframe for a particular field of practice
Six (6) months of recent experience working as a Medical Assistant
Three (3) years of recent health care experience working as a Medical Assistant
Basic computer skills including Microsoft Windows and Office Applications
Preferred qualifications in this position includes:
Experience with electronic medical records and/or computerized billing systems
Typing skills (minimum of 45 words per minute)
