Providence MDS Coordinator in Spokane, Washington
Description:
Providence is calling a MDS Coordinator (Nursing Administration, Days, 1.0 FTE) to Providence St. Joseph Care Center in Spokane, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.
In this position you will:
Collect data for Cognitive, Communication, Vision, Mood/Behavior, PsychoSocial, ADL, Continence, Diseases, Pain, Physical Condition, Dietary, Skin Assessment, Activities, Medications, Special Treatments/Therapies, Home/Discharge plans. Assesses triggers for comprehensive assessments and care plans.
Maintain and periodically update written policies and procedures that govern development, use, and implementation of the Resident assessment (MDS) and care plan.
Perform administrative duties such as completing medical forms, reports, evaluations, and studies as necessary. Function as the weekend on call administrative supervisor on a rotating basis.
Assist in completing care plan portion of the resident’s discharge plan.
Participate in facility surveys (inspections) made by authorized government agencies.
Work with the Interdisciplinary Care Plan Team in developing a comprehensive resident assessment and care plan for each resident.
Serve on, participate in, and attend various other committees of the facility (e.g., Policy Advisory, Quality Assessment and Assurance) as required, and as appointed by the Administrator.
*Must be an active RN or LPN in Washington state
Qualifications:
Required qualifications for this position include:
Education sufficient for licensure
1 year of experience as a MDS Coordinator or case manager in a hospital, long-term care facility, or other related health care facility and 1 year experience with the RAI process in long term care facility
Must have experience in rehabilitative and restorative nursing practices
Must possess a current, unencumbered, active license to practice as a Registered Nurse or Licensed Practical Nurse in Washington state
Preferred qualification for this position include:
- 1 year of experience in an acute care or long-term care facility

Providence St. Joseph Care Center in Spokane, Washington is located on the campus of Providence Sacred Heart Medical Center & Children's Hospital. Our Transitional Care Unit is designed for patients coming out of the hospital who need skilled nursing care, end of life care, or rehabilitation for those recovering from surgery or who have a functional loss from illness, disease, or an accident. Our Long Term Care unit provides skilled nursing in a residential setting, addressing the needs of our residents through creating a sense of community and home and providing a model of care that is resident-centered.
