Social Worker MSW - As part of the Health and Well-Being Team for the Sisters of Providence (SP), in collaboration with the Dominican Sisters of the Order of Preachers of Tacoma (Op-T), provides care management and assistance to the Sisters of both religious communities primarily residing in Western Washington. Consults and advises regarding healthcare needs and services. Performs all essential functions of care coordination as needed. Enhances sister's understanding of her own medical and psychosocial needs.This position sits half time from the Providence West Seattle location and half time from the Providence Tacoma location.
Required qualifications:
Master's Degree in Social Work, with concentration in Medical/Healthcare Social Work settings, or equivalent.
5 years Experience in home health and/or care coordination or equivalent.
Demonstrated work experience with women’s health issues and concerns.
Demonstrated experience coordinating community resources in areas served.
Washington Advanced Social Worker License (Vendor Managed) Or
