We are seeking a Community Health Worker III - Homeless Services who will be responsible for helping community residents and Medical Center patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW-III will provide job duties within the following track:

Navigation/Linkage to Resources: CHWs in this track will manage complex patient sub-populations, specifically patients experiencing homelessness. The CHW-III will be paired with a Social Worker to manage these individuals and they will provide individualized assessments of risk factors that impact an individual's well-being. They will be responsible for doing an assessment in community and Medical Center Emergency Department settings, then referring them to the appropriate programs and resources in the community, and providing follow-up to make sure the needs are met.

In this position you will:

  • Develop and maintains a positive and professional relationship with community stakeholders that promotes support for continued involvement and cooperation in community health projects

  • Provide quality service in the performance of work assignments and duties

  • Navigation/Linkage to Resources Duties (for CHWs in the navigation/linkage track)

  • Manage complex patient subpopulations, specifically patients experiencing homelessness. This can include but is not limited to people with co-morbidities, recently discharged patients from the Medical Centers and/or community residents struggling with substance abuse

  • Acquaint homeless individuals with community resources and encourages the utilization of these community services

  • Work as the primary lead on completing a safe and seamless discharge for the homeless patient population out of the Emergency Department in accordance with the provisions set forth State legislation (ie SB1152). This involves following Providence policies and procedures that were established to implement SB 1152

  • Work with homeless individuals to find the most appropriate shelter or recuperative placement that is best suited to patient specific needs

  • Assist the homeless with accessing primary care, mental health care, and substance use disorder treatment. Assist individuals with navigating health care and other systems

  • Provide support to individuals struggling with behavioral health issues and homelessness. Utilize motivational interviewing and harm reduction techniques

  • Act as liaison between housing agencies and other service providers in the community and seeks out other community resources that would benefit this population

  • If warranted, will assist with completing housing/homeless service providers applications or registration forms for services; may complete client demographic logs and other statistical information

  • Work collaboratively and professionally with Social Worker assigned to ED, when necessary to manage complex patient subpopulations

  • Provide on-site screenings of risk-factors that impact an individual’s well-being in community and Medical Center settings and provides resources for identified risk factors

  • Build relationships with clients/patients by asking appropriate questions in a compassionate manner and uses good listening skills

  • In Medical Center settings will refer patients to appropriate CHW who can enroll patients into Hospital Presumptive Eligibility (HPE) and provide enrollment assistance or refer patients to CHIP for patients that need to be enrolled in Covered CA or CalFresh

  • Follow up and assist families who are having difficulty accessing resources to which they were referred

  • Work closely with Community Health and Medical Center caregivers to ensure a seamless flow for the patients

  • General Outreach Duties

  • Actively seek opportunities for outreach with a focus on hard to reach populations and our identified “high-need” communities

  • Conduct presentations about available services to local community based organizations

  • Maintain and strengthen contact with key stakeholders related to program updates

  • Work with other caregivers within Community Health Department (e.g. Outreach Specialist, other Supervisor, etc.) to coordinate outreach events and direct service opportunities and to ensure Outlook calendar is updated with event information (time, type, location, assignments)

  • While performing outreach activities, recognizes possible community resources and brings back information to supervisor

  • Maintain knowledge of Providence Community Health programs and provides community with appropriate linkages as requested.

  • Act as representative for Providence Health and Services at professional and network meetings, as needed

  • Attend and participate in regular staff (Community Health and Care Management) and department-wide meetings, as recommended by Supervisor

  • Attended CHW trainings, as required by supervisor, to stay up-to-date on programs

  • Documentation/Information Management

  • Document all outreach events and direct services in Electronic Health Record, including maintenance of logs and sign-in sheets

  • Implement and monitor collection of program’s evaluation data (e.g. surveys)

  • Ensure that program statistics are entered into the appropriate database (e.g. Carescope. LAHSA, EMR) or other Community Health database per department deadlines

  • Compile and review monthly reports related to expenses and service units

  • Submit monthly reports to Supervisor and/or Department Data Specialist

  • Development

  • Develop, maintain and demonstrate up-to-date knowledge on community health issues by attending seminars, trainings, or conferences as required by the Department, reading materials or other methods

  • Develop, maintain, and demonstrates up-to-date knowledge of hospital and community resources

  • Stay up to date with current literature to ensure curriculum is based on best practices

  • Participate in Community Health staff development and/or training activities for CHW’s


Required qualifications:

  • Valid Driver's License

  • 1 year experience in a combination Community Health Worker I and II position, or 2 years related community outreach, enrollment, education or other community health administrative experience

  • Ability to work collaboratively with others and to participate fully in a team process

  • Strong public speaking skills

  • Excellent interpersonal skills

  • Ability to complete necessary tasks and meet deadlines.

  • Ability to follow directions, be a self-starter and collaborate with coworkers.

  • Willingness to learn computer systems and programs

  • Accurate word processing/typing skills

  • Experience dealing with behavioral health issues and/or navigating the homeless system preferred.

  • Experience developing relationships with individuals with serious mental illness and/or substance use disorder, and a demonstrated ability to connect with and support this population

  • Ability to manage multiple priorities concurrently in a fast paced environment with excellent organizational skills

  • Ability to communicate effectively with homeless individuals, treatment providers, homeless provider staff.

  • Must be self-driven and able to function independently

Preferred qualifications:

  • 2 years working in similar field as a community health worker, peer specialist, peer advocate, lived experience of homelessness or a similar role preferred and/or volunteering in the community highly preferred

  • Bi-lingual English /Spanish

  • Familiarity with Project Roomkey

  • Credentials to work with the homeless population
