Providence Health & Services Hospice Clinical Manager in Torrance, California

Description:

Providence is calling a Hospice Clinical Manager (Full time/Day shift) to Providence TrinityCare Hospice in Torrance CA.

We are seeking a Hospice Clinical Manager who will be responsible for ensuring that patient and family care is coordinated, delivered and managed appropriately 24 hours/day, 7 days/week. This position will be responsible for the day to day clinical management and supervision of the team's clinical and non-clinical staff. The Hospice Clinical Manager will ensure effective collaboration and communication with internal and external customers and healthcare delivery systems that impact the day to day care delivery. May provide care to premature infants, infants, pediatrics, adolescents, young, middle and older adults.

In this position you will:

  • Receive and facilitate patient referrals and admissions to hospice services

  • Review available patient information related to case, including home visits, to determine hospice/home care needs

  • Assign appropriate hospice personnel to case as needed

  • Confer with Hospice Medical Director regarding any questions about an individual’s eligibility for services

  • Comply with accepted PH&S and CHAP professional standards and principles

  • Maintain productivity standards set by organization guidelines

  • Review and evaluate each admitted patient/family through a variety of means: home visits, care conferences, IDG/IDT meetings, clinical record reviews and the services provided by clinicians and non-clinicians. Uses the information to guide, develop, mentor and motivate team clinicians to promote optimal performance and delivery of the highest quality hospice services

  • Available at all times during operating hours, and on call per agency policy, to assist clinicians as appropriate

  • Assist/Mentor all team disciplines in establishing appropriate, timely, well collaborated Plans of Care

  • Complete annual supervisory visits and competency evaluations on all direct reports

  • Ensure total compliance with monthly supervisory visits for LVN and CHHA staff

  • Coordinate weekly Interdisciplinary Group (IDG/IDT) meeting for the review and revision of each patient’s Plan of Care every two weeks

  • Screen, interview and hire new team clinical and non-clinical staff

  • Schedule and coordinate after hours, weekends and holiday RN on-call coverage for team caseload

  • In collaboration with the Chief Nursing Officer, evaluate and analyze risk reports, internal/external quality variance reports and take appropriate and timely follow-up action when indicated

  • Evaluate the care delivery of the team(s) by regular review of specified Management Reports, direct observation, clinical record audits, family evaluation of hospice care (FEHC) survey results and other management data to ensure the delivery of the highest quality services

  • Participate in Monthly Operating Review (MOR)/Leadership Team meetings and activities and other management meetings as requested by Chief Nursing Officer

  • Participate in managing team expenses to cost/day budget expectations

  • In collaboration with the Associate Director of Provider Relations, establishes and develops professional relationships with key referral sources and healthcare partnership

  • Review patient’s medical diagnosis, prognosis, medications, procedures and clinical course

  • Conduct regular clinical record audits and other documentation performance improvement activities as assigned

  • Communicate findings of audits and activities with recommendations to the Chief Nursing Officer and the Associate Director of Quality Management

  • Perform Follow-up activities in a timely manner on all formal Quality Variance and Risk Reports, communicating findings and recommendations to the Chief Nursing Officer and the Associate Director of Quality Management

  • When making any patient contact, complete documentation in the clinical record within the timeframe set by the organization guidelines

  • Coordinate the orientation, education and development of clinical and non-clinical staff

  • Assist in the formulation, review, revision, implementation and evaluation the organization’s policies, procedures, goals and objectives

  • Comply with Hospice Policy and Procedure Manual, Hospice Safety Manual, PH&S Employee Handbook and PH&S Human Resources Policy Manual

  • Comply with patient care requirements of all regulatory agencies and accrediting bodies

Qualifications:

Required Qualifications:

  • Associate's Degree in the applicable clinical discipline

  • California license as a Physical Therapist, Occupational Therapist, Speech Language Pathologist, or Medical Social Worker

  • Basic Life Support certification

  • Current CA Driver license

  • Automobile insurance

  • 1 year previous experience in the applicable discipline in the hospice setting

Preferred Qualifications:

  • Bachelor's Degree in the applicable clinical discipline

  • 3 years supervisory/management experience
