Providence Health & Services Lead Licensed Clinical Social Worker (LCSW) in Torrance, California

Description:

Providence is calling a Lead Licensed Clinical Social Worker (LCSW) (Full time, Variable Shift) to Providence Little Company of Mary Medical Center in Torrance, CA.

We are seeking a Lead Clinical Social Worker (LCSW) who will perform all duties of the Licensed Clinical Social Worker, including but not limited to, providing psychosocial assessments, counseling, information and referral to inpatients and outpatients and their families, while working collaboratively with medical and paramedical staff. The Lead Licensed Clinical Social Worker will also oversee the daily work operation of the Clinical Social Workers and train staff as needed. In conjunction with the Director or Manager of Case Management, The Lead Licensed Clinical Social Worker is accountable for input regarding personnel management and ensuring the timely delivery of supportive professional services to patients, families and hospital staff.

In this position you will:

  • Promote adherence to the Company's Code of Conduct and the Corporate Compliance Agreement by monitoring employee performance and identifying, responding to compliance issues and reporting any such issues to the Director or Manager of Case Management

  • Maintain quality, patient satisfaction, and preservation of patient rights and dignity

  • Lead the daily workflow of staff clinical social workers; assisting with problem solving with escalated issues or concerns, attendance/staff scheduling, etc

  • Formulate, implement, and document goal-directed care plans based on patient/family needs, outcomes and evidenced based research

  • Provide staff development and training

  • Collaborate with other services lines to promote quality patient care

  • Provide assessment, guidance, information and referrals to hospitalized patients, outpatients and their families

  • Designs, facilitates and/or implements, in conjunction with the Director or Manager, quality improvement projects to improve patient care process/systems within the department

  • Incorporate results of customer and staff surveys into quality improvement projects

  • Promote and develop relationships with physicians, facilitates good working relationships between physicians and staff, and maintains a high level of professionalism

  • Participate in the hospitals service and education programs

  • Plan, evaluate and interpret statistical and narrative analytical reports of department services relative to patient needs

  • Provide input with developing the unit operating budget, assists in maintaining necessary controls to assure compliance with budgeted expenses, resources and staff projections

  • Provide input in selecting new employees by participating in the interviewing process

  • Evaluate the progress of social work staff and submits recommendations for staff commendation or disciplinary actions

  • Perform assigned functions independently, exercising critical thinking and consistently good judgment

  • Provide crisis coverage and on-call duties as assigned

  • Assist with utilization review, peer review, and performance improvement processes within the department

  • Perform child, elder, and domestic abuse reporting as required

  • Identify appropriate community resources and deals effectively with them to facilitate services for Providence Little Company of Mary Medical Center patients/families

  • Participate in interdisciplinary patient planning and takes leadership assignments, i.e. educational presentations to staff

  • Demonstrate competency in selected skills including, but not limited to:

  • Crisis intervention

  • Psychosocial assessment

  • Individual and family counseling

  • Conduct support and process groups

  • Provide patient/family education, including information and referral

  • Provide consultation for medical, nursing, and ancillary staff regarding patient/family psychosocial issues

  • Demonstrate responsibility and accountability for own performance and departments performance in regards to:

  • Attendance and punctuality

  • Positive public relations image and working relationships

  • Completing all mandatory requirements including current licensure, annual physicals, etc.

  • Meeting regulatory compliance standards (e.g. TJC, Title 22, California Hospital Association (CHA), California Probate Code and HIPPA)

  • Comply with Medical Center, department and unit standards, including but not limited to safety, infection control, quality management, environment of care, confidentiality, and patient care protocols and procedures

  • Obtaining and maintaining necessary knowledge and skill needed to maintain competence

  • Participates in the development and attainment of team, department, and Medical Center goals, including but not limited to:

  • Attending and participating in team meetings

  • Identifying opportunities to improve or enhance team, department, or Medical Center’s performance, and bringing such opportunities to the attention of leadership

  • Participates in committees required

Qualifications:

Required qualifications for this position include:

  • Master's Degree from an accredited School of Social Work

  • Licensed Clinical Social Worker in California

  • Five (5) years experience as a Master of Social Work

  • Four (4) years experience in health care setting

  • Knowledge of mandatory reporting requirements

  • Knowledge, use and supervision of crisis intervention procedures

  • Knowledge of community resources available to all patients

  • Knowledge of HIPAA standards compliance and confidentiality practices related to all patients and/or employee information to assure patient and/or employee rights and protected

  • Demonstrated knowledge and skills necessary to provide care appropriate to patients on any assigned unit

  • Excellent communication and organizational skills

  • Knowledge of the principles of growth and development and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify the requirements of each patient

  • Able to prioritize, organize and coordinate unit activities

  • Knowledge of legal, ethical, risk management, and safety issues involving social work activities

Preferred qualifications for this position include:

  • Four (4) years Supervisory/management experience in a social agency
