Providence is calling an LVN Scheduler (Full-time / Day Shift) to Providence TrinityCare Hospice in Torrance, CA.

We are seeking a LVN Scheduler who will be responsible for coordinating the daily, weekly, and monthly assignments for Registered Nurses, Home Health Aides, MSW Care Coordinators and Chaplains; and on a rotating schedule providing administrative support to the Triage and After Hours Clinical Support department staff.

In this position you will:

  • Provide an accurate daily schedule of clinical visits for hospice services:

  • Schedule additional visits as patient needs develop; coordinate schedules taking into consideration caseload availability and staffing levels and geographic areas and expertise to maximize efficiency, safety and quality of care

  • May assist Triage staff and/or RN Case Manager to adjust scheduled visits when staff call in sick or are on vacation

  • Support and update staff schedules; communicate with staff on a regular basis regarding changes in schedules, changes or variations in coverage needs, patient volumes, staffing levels to ensure efficient, effective and consistent patient coverage

  • Communicate scheduling needs and coverage to managers; provide input and observations regarding the day-to-day function of the department to Managers

  • Participate in planning and implementing activities to accomplish the department goals and objectives

  • Maintain confidentiality of all ministry, company, and patient information. Ensure proper handling and disposal of confidential documents as outlined in applicable Providence polices, and HIPAA regulations

  • On a rotating schedule, provide administrative support to the Triage and After Hours Clinical staff, including the following:

  • Provide coverage of incoming telephone calls, appropriately addressing caller’s needs or routing them correctly to meet the patient’s needs

  • Provide administrative support, including entering data into clinical system applications, ordering Durable Medical Equipment (DME) and coordinating delivery and pick up processing, data tracking processes for quality projects, and maintaining staff schedules and time off/on-call calendars

  • Assist nursing staff in contacting pharmacies regarding availability of medications, prescriptions status, hours of operation, and maintain pharmacy resource list. Arrange couriers and taxi delivery of medications when instructed by triage nurse

  • Run McKesson reports as scheduled and directed and distributes as required

  • Contribute to overall team functioning through effective time management and utilization of problem-solving skills to address team issues:

  • Appropriately utilize supervisor for guidance/problem solving, including complaint issues and concerns

  • Attend staff meetings, quality improvement activities, and in-service training sessions assigned

  • Participate in new staff or volunteer orientation and training as appropriate

  • Primarily, the LVN scheduler will be coordinating admission visits with Liaisons, Access Services, Clinical Managers and Registered Nurses including:

  • Deciding on what palliative treatments are appropriate in a plan of care

  • Working with the HUB on goals of care conversations for admission

  • Moving RN schedules around to accommodate urgent needs within the skill set of the RN

  • Deciding which patients need urgent admissions due to symptom management needs

  • Assessing for safety concerns prior to sending out staff


Required Qualifications:

  • Licensed Vocational Nursing

  • Current California State licensure as a Licensed Vocational Nurse (LVN)

  • California Current Driver's license and automobile insurance

  • Current CPR License

  • Ability to communicate in English effectively both verbally and in writing; demonstrated knowledge of phone etiquette

  • Demonstrate competency by maintaining positive, collaborative and constructive interpersonal relationships; understand and practice the principles of effective teamwork

  • Demonstrate ability to prioritize work assignments and meet productivity and quality standards

  • Adapt easily to changes in work assignments and environments

  • Manage scheduling admissions, deciding on what palliative treatments are appropriate in a plan of care, working with the HUB on goals of care for admission

  • Moving RN schedules around to accommodate urgent needs

  • Deciding which patients need urgent admissions due to symptom management needs

  • Assessing for safety concerns prior to sending out staff

Preferred Qualifications:

  • Hospice certification

  • 1 year nursing experience

  • 1 year prior Hospice, Homecare, Long Term Care, medical/surgical, and/or oncology experience

  • Ability to use standard office equipment including personal computers, telephones, copiers and fax machines; proficiency with MS Office products including Word, Excel and Outlook
