We are seeking a Nurse Navigator who will coordinate the continuum of care for the patient diagnosed with cancer and maintains communication between the primary physician, nursing staff, consulting physician, and ancillary departments. Responsibilities include coordinating this inter-professional team of resource providers to best serve the patient with cancer: provide education and information to the patient and his/her family, support the patient during the difficult decision-making period, coordinate, as needed, multiple physician appointments, identify appropriate resources and provide emotional support.

Establish and maintain communication between hospital departments and services for the purpose of problem-solving and easing the way for patients utilizing these services
Facilitate understanding of physician recommendations for patient care to help ensure patient compliance
Provide department education as needed.
Meet with patients early in the diagnostic process to provide education, emotional support, and answer questions for the patient and/or her family or support persons
Act as an advocate for cancer patients and their families
Assist in formulating a summary of a patient's first course of treatment and follow-up care (Survivorship Care Plan)
Facilitate delivery of the Survivorship plan to the patient providing education and answering or directing questions as appropriate.
Support and navigate patients through the challenges related to each stage of their disease, including the end-of-life process
Assess patient educational and social needs and makes appropriate referrals
Synchronize, as needed, multiple physician appointments and identifies appropriate resources for the patient
Coordinate and support use of ambulatory and supportive care services recommended for the patient including social services, nutritional counseling, genetic risk assessment/counseling, rehabilitation and others as available/needed.
Act as primary resource for patients with cancer. Available to each and every patient being followed by phone, email, or appointment for any support questions
Build relationships with referring physicians and implements appropriate protocols
Be responsible for initiating oral and written communications to the appropriate person when problems arise
Be responsible for communicating observations to appropriate members of the interdisciplinary health care team.
Facilitate and conduct educational programs/seminars to community organizations as well as hospital staff
Develop and maintain working knowledge of clinical research protocol criteria
Promote and support opportunities for participation in clinical trials
Utilize medical records to obtain and document pertinent patient information
Track and monitor diagnosis and treatment information, outcomes, and patient contacts for measuring and improving quality of care for the individual patient and patient population
Maintain current working knowledge and understanding of the oncology field. Enhances professional growth and development through participation in educational programs, professional affiliations, current literature, in-service meetings, and workshops
Required qualifications for this position includes:
High School Diploma - or- GED
Graduate of an accredited Registered Nursing Program.
Bachelor of Science, Nursing
Current CA RN Licensure
Basic Life Support (BLS) for healthcare providers.
One of the following certifications: OCN, AOCN, AOCNP, CBCN required within one (1) year of hire
Three (3) years of experience in related area
Knowledge of the oncology disease process, treatment protocols for chemotherapy and radiation therapy, treatment side effects and outcome for process
Ability to support patients and families and support persons in the end-of-life process and provide compassionate supportive care.
Aptitude to collect, organize and analyze data, synthesizing it into understandable information
Ability to work independently, triage patients and keep physicians informed of patient status.
Program development experience highly desired
Experience in public speaking/leading educational programs is highly desired
Knowledge of computer applications as appropriate to the role
Customer Service Skills
Preferred qualifications for this position includes:
Bachelor's Degree in Nursing
Master of Science, Nursing
Three (3) years of Oncology experience and strong interest in the care of the oncology patient.
Two (2) years outpatient or medical practice experience, Care management experience
