We are seeking Palliative Care MSW who will participate in the activities of the interdisciplinary Palliative Care Team. The PC MSW participates in the development and review of appropriate protocols, facilitates formal and informal meetings regarding patient and family communication and decision making processes that support appropriate referral and consistent care, works in collaboration with physicians, hospital, home health and hospice health care teams to provide Palliative Care services in patient, out-patient and in home settings to seriously ill patients and their families. The PC MSW engages in independent decision-making about psychosocial needs, provides psychosocial services to individuals and their families, and provides formal and informal meetings regarding patient and family communication and decision making. The PC MSW obtains a psychosocial history, including chart review and information gathered from the patient and family, and conducts psychosocial palliative care needs assessments. The PC MSW provides direct patient care as well as consultative services to assure the delivery of state-of- the-art palliative care. In addition to these clinical duties, the Palliative Care MSW is responsible for contributing social services work expertise to staff education, data collection, and quality improvement activities as they relate to inpatient, outpatient and home palliative services.
In this position you will:
Assess psychosocial needs for specific patient populations using advanced practice palliative care and hospice knowledge and skills:
Obtain psychosocial history
Identify patient short and long term goals
Identify available family and social support
Initiate advance care planning discussion
Identify discharge options and ongoing care needs related to patient diagnosis, prognosis and identified care giving needs
Provide critical psycho-social support to palliative care patients and their families through the palliative care services consult team
Use their professional background and skills to assist the team and facilitate palliative care patient-family meetings, and assists team, attending physicians, staff, and patient and family in the clarification of the palliative goals and plan of care
Assist the Palliative Care Director in ensuring excellent palliative care patient care services, coordinating educational and outreach services, leading quality improvement initiatives, and linking team efforts with other health system departments including continuum and community services
Synthesize information from the psychosocial history and needs assessment to develop, document and implement the psychosocial dimension of the holistic palliative care treatment plan
Develop and implement treatment plan in collaboration with members of the Palliative Care Team,
Collaborate, as needed, with patient’s primary physician, consulting physicians and other health care team members in developing realistic treatment goals
Communicate effectively with patients and families for understanding of diagnosis, prognosis, and plan of care
Determine effectiveness of treatment plan and documents patient care outcomes
Revise and modify the treatment plan to achieve patient goals
Present hospice, home health and continuum information effectively
Collaborate with acute, hospice, facility and home care MSWs to assure continuity of psychosocial support for patients and families discharged to home or facility, on home care or hospice
Understand the psychosocial needs of frail patients with advanced illness
Meet setting of care and JCAHO/CHAP documentation standards
Assist program director in the development of assessment tools and program-specific policies and procedures
Respond in a timely manner to meet physician and patient needs
Coordinate Palliative Care Team efforts with appropriate hospital support departments to ensure smooth discharge planning and clear follow-up post discharge; may assist in the development and implementation of creative community or system care options for patients
Assist Director as a liaison between acute ministries, outpatient and home palliative services, Providence TrinityCare Hospice and other community hospices, home health services, and nursing homes to ensure care coordination and appropriate assignments and resource allocation to meet patient needs throughout the continuum of care
Promote palliative and hospice care to patients, families, community and other health care professionals:
Serve as a palliative psychosocial resource to hospital and continueem staff and team members
Develop staff and physician relationships that facilitate ongoing palliative care referrals
Provide education to palliative care patients, families or caregivers as needed
Provide continuing palliative care clinical education to staff, community and other professional groups as requested
Required qualifications for this position includes:
Master's Degree in Social work or equivalent
Current Driver's License and Auto Insurance
Three (3) years Inpatient, Outpatient, or home-based Social work
Demonstrated evidence of experience in palliative care settings, such as hospice, i.e., ability to:
Work independently as well as collaboratively on a small interdisciplinary team
Provide and model interdisciplinary palliative assessment and management of physical, psychosocial and spiritual symptoms during aggressive treatment and end of life care
Possesses a working knowledge of comprehensive palliative care
Knowledge of MSW scope of practice
Knowledge of stages of dying/death and age-appropriate outcomes and interventions
Knowledge of basic pain and symptom assessment
Knowledge of infection Control principles and practice
Understands role as a clinical leader
Functions well with an interdisciplinary team
Time management skills.
Critical thinking and prioritization skills
Communication and interpersonal skills
Knowledge of referral networks
Knowledge of quality improvement processes
Knowledge of cultural differences and culturally appropriate outcomes and interventions

Preferred qualifications for this position includes:
LCSW or equivalent
Working knowledge of research techniques, data collection, and policy, program and curriculum development
Experience with quality improvement methodology
