Providence Health & Services Nurse Liaison - Home Infusion Therapy in Tukwila, Washington
Providence is calling a Float Nurse Liaison (Home Infusion Therapy, Day Shift, 1.0 FTE) to Providence Infusion and Pharmacy Services around the King County Metro in Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this position you will:
Practices in accordance with Providence Infusion and Pharmacy Services’ philosophy, mission and code of conduct
Provides both telephone and on-site information and consultation to assigned hospital regarding home infusion services as appropriate for patients, families, facility/agency physicians, and hospital social workers/discharge planners.
Evaluates referral received on hospitalized patient for appropriateness for hospice, home health and/or home infusion and other in-home services programs as appropriate.
Maintains open positive communication with appropriate agency staff (hospice, home health, and/or home infusion, etc.) regarding referral received.
Facilitates smooth transition of patient from hospital to agency services to provide the best continuum of care. Could include patient education, teaching of equipment use, technical support, and active participation in the discharge process as appropriate.
Admits and educates patients/caregivers in all infusion therapies
Introduces services to hospitalized patients and their families.
Required qualifications for this position include:
Graduate of accredited school of nursing
One (1) year experience in acute care nursing
Current Washington State Registered Nursing License
Current Washington State Driver's license and automobile insurance
Current CPR certification
Preferred qualification for this position include:
Home health or home infusion experience
Prior sales experience
