Providence Pharmacy Technician in Tukwila, Washington
Providence is calling a Pharmacy Technician (1.0 FTE, Day Shift) to the Pharmacy Department at Providence Infusion and Pharmacy Services in Tukwila, Washington. This position will require working out of clinics at Providence ElderPlace in Seattle, West Seattle, Kent, and Redmond, WA.
Position Summary: This position is responsible for supporting Elderplace care providers, team members and clinical pharmacists in coordinating and providing clinical pharmacy services to Elderplace participants. The clinical pharmacy technician contributes to the care management of Elderplace participants by sharing knowledge about orders, medication delivery, caregiver response to instructions and coordination of medications when living environments change. Also provides appropriate caregiver education as needed, within the scope of this position, for medications and orders. The clinical pharmacy technician also assists with medication reconciliation prior to onboardings and assessments.
In this position you will: .
Serve as a liaison between pharmacy staff and Elderplace nurses, PCPs, and pharmacists in any medication or prescription related issues or discrepancies, utilizing effective communication and collaboration skills to ensure safe and consistent practices.
Gather prescription information by interviewing patients, caregivers, pharmacies, or reviewing hospital records, MARs, EMRs and Epic Care Everywhere to gather information about current and past medications, labs, prior visits, etc. This includes drugs, doses, refill dates and quantities, and Dx/indication. This may include collecting information on tablet markings to give to the clinical pharmacist for definitive identification of medications, identifying congruencies and discrepancies between various sources of information and report this to the pharmacist or providers, and gathering information for Mediset requests.
Monitor Epic message pools to identify medication changes, moves, new orders, electronic rx renewal requests, failed faxes, escribe errors, etc.
Coordinate refills for all Elderplace participants by taking phone calls, voicemails, emails, faxes and regular mail from AFH, ALF, SNF, patients and other caregivers (including IDT members and providers) requesting refills of medications. Assess if current utilization matches prescription. If not (e.g. request to refill inhaler 2 weeks early or 2 months late) notify pharmacist or provider.
Forward rx renewal requests from Providence Oral Dose Pharmacy or other outside pharmacies to the appropriate provider or pharmacist to review via Epic message or email.
Alert provider if refill language allowing for CDTA is missing in Epic within the past year.
Assist in arranging for participant moves by collecting MARs or prescription information for medication reconciliations.
Coordinate delivery of medications by providing information on the status of a delivery (via courier or USPS), coordinate pick-up of medications from one location to another, or authorize courier or stat courier of medication.
Print and send medication letters for new, changed, or discontinued medications using Epic smart phrases and route to AFHs, ALFs, SNFs, and patient homes via USPS mail service, fax, or secure email.
Print and send MAR on request via USPS mail service, fax, or secure email.
Answer questions not requiring professional judgment (e.g. cost, availability of medications, refill dates, status of delivery, etc.) and triage patient questions, referring any requiring clinical or professional judgement to the clinical pharmacist, provider, or nurse case manager (RNCM).
Clarify orders by transferring information from provider email or Epic encounter into pended Epic order ONLY if it does not require clinical judgment (e.g. clarify tablet size [e.g. 1x50mg tablet rather than 2x25mg], remove unsafe abbreviations, update Rx to current therapy [e.g. remove taper orders once pt on stable dose]) or adjust quantities to match manufacturer’s package size.
Communicate to Elderplace provider the need for review or orders written by non-PEP providers.
Transcribe information from provider using Epic, email or fax and pend a medication order in Epic ONLY if it does not require clinical judgment (see clarify orders). Pended orders will require clinician sign-off before implementation.
Notify appropriate clinic LPN or RNCM of supply requests for dispense from clinic stock or direct order from Medline.
Review MARs for Elderplace participants residing in ALFs or AFHs on behalf of the RNCM with each semi-annual and annual assessment for any medication discrepancies and report to the care team for review.
Obtain medication lists from previous pharmacies or MARs from current ALF or AFH for new enrollees to the Elderplace program and pend to the clinical pharmacist prior to enrollment date for review.
Monitor EPCS daily reports via Infoview for Epic log in failures by the providers and report 5 or more to the medical director for follow up
Assist with provider, care team, or caregiver education relating to pharmacy services and procedures.
Assist with standardization projects across different Elderplace clinical sites and care teams.
Required qualifications for this position include:
Completion of approved pharmacy technician training program
Current certification and license by the Washington State Board of Pharmacy as a Pharmacy Technician
Current National Pharmacy Technician certification, or within 6 months upon hire
Preferred qualifications for this position include:
Customer service experience
Experience using QS1, Docutrack, and Epic
