Providence Health & Services RN Wound Care Specialist - Home Health King County in Tukwila, Washington
Providence is calling a Registered Nurse (RN) ( Home Health King County, Day Shift, 1.0 FTE) to Providence Home Services of King County in Tukwila, Washington . In this position you will:
Serve as a clinical resource to agency staff on issues pertaining to patients with wounds, pressure ulcers, ostomies (if area of expertise), fistulas and incontinence (if area of expertise). Makes joint visits for evaluation and education purposes as needed.
Perform comprehensive assessments, develop plans of care, coordinate care with other disciplines and provide direct patient care in the implementation of outcome oriented interventions under the physician ordered Plan of Care.
Perform OASIS comprehensive assessment primarily for patients with wounds and ostomies.
Promote and deliver coordinated wound, ostomy, and continence services per certification held.
Promote comprehensive care for patients through coordination of medical, agency, and community resources. Assists patients with preparation for discharge including supplies needed.
Collaborate with all healthcare team members (internal & external) toward the best outcomes for the patient and ministry.
Assess POC of ostomy patients (if area of expertise) and promotes self-care and psychosocial rehabilitation. Evaluates the effectiveness of care provided and revises care plan if indicated.
Serve as an educator to patients and their families, staff, physicians, and other healthcare professionals in the community regarding the management of ostomies, fistulas, wound, pressure ulcers, and incontinence (up to level of certification). This may include in-service education and orientation classes.
Required qualifications for this position include:
Coursework/Training Graduate of a certification program provided by Wound Ostomy Continence Nursing Certification Board (WOCNCB). Or
Coursework/Training Graduate of a certification program provided by National Alliance of Wound Care and Ostomy (NAWCO). Or
Coursework/Training Graduate of a certification program provided by American Board of Wound Management (ABWM).
2 years RN experience.
Experience managing simple and complex wounds.
Current certification in one of the following: CWOCN (Certified Wound Ostomy Continence Nurse), WOCN (Certified Wound Ostomy Nurse), CWCN (Certified Wound Care Nurse) , WCC (Wound Care Certified) , CWS (Certified Wound Specialist), CWCA (Certified Wound Care Associate).
Current physical therapist Washington State License.
Professional scope of practice allows for wound care in the state of Washington.
Current Driver’s license and have access to daily use of an insured, reliable vehicle.
Current Provider BLS certification.
Preferred qualifications for this position include:
Experience in a Medicare-certified Homecare or Hospice agency/facility.
Experience managing and teaching ostomy care.
2 years Direct-patient experience with wound certification.
