Providence Senior Community Health Worker in Van Nuys, California

The Community Health Worker III (CHW-III) is responsible for helping community residents and Medical Center patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW-III provides primary job duties within at least one of the following tracks:

a) Health Education: CHWs in this track will assist with recruitment and administrative tasks for health education courses, such as but not limited to Mental Health First Aid, Creating Healthier Attitudes Today, physical activity, or Groceryships. In addition they will deliver these modules these health education topics independently and identify new locations where the programs can be delivered.

b) Outreach and Enrollment: CHWs in this track will provide outreach in community settings and provide initial screening for eligibility of Covered CA, Medi-Cal, and CalFresh (Food Stamps). In addition they will provide one-on-one health insurance and/or CalFresh enrollment application assistance. In addition they will also troubleshoot complex cases independently and provide small group presentations on how to use insurance and basic health care rights.

c) Navigation/Linkage to Resources: CHWs in this track will manage complex patient subpopulations, including but not limited to people with co-morbidities, recently discharged patients from the Medical Centers or community residents struggling with substance abuse. When necessary, the CHW-III will be paired with a Social Worker to manage these individuals. As part of this they will provide individualized assessments of risk factors that impact an individual's well-being. They will be responsible for doing the assessments in community and Medical Center Emergency Department settings, then referring them to the appropriate programs and resources in the community, and providing follow-up to make sure the needs are met.

Required Qualifications:

  • Education to meet certification, license or registration requirement

  • Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.

  • 1 year experience in a combination Community Health Worker I and II position, or 2 years related community outreach, enrollment, education or other community health administrative experience, or Bachelors degree in lieu of experience

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
