Providence is calling a Social Worker Complex Care (LCSW) (Full-Time/Day Shift) to Providence Medical Institute in West Hills , CA.

We are seeking a Social Worker Complex Care (LCSW) who will provide social work services to patients/clients in a variety of residential settings to enable patients and significant others to more effectively deal with the impact of medical and/or psychiatric illness, or disability, on individual and family functioning. Assist in the finding of satisfactory solutions or adjustments to medical, social, emotional and financial problems related to illness or disability which may interfere with access to and use of medical care, taking into account patients'''' physical or age-related needs. Participates in the development and review of appropriate protocols, facilitates formal and informal meetings regarding patient and family communication and decision making processes that support appropriate referral and consistent care. Works in collaboration with physicians and hospital and hospice health and interdisciplinary care teams to services to seriously ill patients and their families. Engages in independent decision-making about psychosocial needs, provides psychosocial services to individuals and their families, and provides formal and informal meetings regarding patient and family communication and decision making. Obtains a psychosocial history, including chart review and information gathered from the patient and family, and conducts psychosocial assessments. Provides direct patient care as well as consultative services to assure proper referral to Complex Care Management, Palliative Care, Hospice or other community resources. In addition to these clinical duties, is responsible for contributing social services expertise to staff education, data collection, and quality improvement.

In this position you will:

  • Assess psychosocial needs for specific patient populations using advanced practice palliative care and hospice knowledge and skills:

  • Obtain psychosocial history

  • Identify patient short and long term goals

  • Identify available family and social support

  • Initiate advance care planning discussion

  • Identify discharge options related to patient diagnosis, prognosis and identified care giving needs

  • Provide critical psycho-social support to patients and their.

  • Use their professional background and skills to assist the team and facilitate palliative care patient-family meetings, and assists team, attending physicians, staff, and patient and family in the clarification of goals and plan of care

  • Assist in ensuring excellent palliative care patient care services, coordinating educational and outreach services, leading quality improvement initiatives, and linking team efforts with other hospital departments and with outpatient and community services

  • Assist in coordination efforts with appropriate hospital support departments to ensure smooth discharge planning and clear follow-up post discharge; may assist in the development of creative community or system care options for patients

  • Assist Supervisor/Directors a liaison between Medical Group and Foundation, HBPC service team, Providence TrinityCare Hospice and other community hospices, home health services, and nursing homes to ensure care coordination and appropriate assignments and resource allocation to meet patient needs throughout the continuum of care

  • Synthesize information from the psychosocial history and needs assessment to develop, document and implement the psychosocial dimension of the holistic palliative care treatment plan

  • Develop and implements treatment plan in collaboration with members of the Palliative Care Team

  • Collaborate, as needed, with patient’s primary physician, consulting physicians and other health care team members in developing realistic treatment goals

  • Communicate effectively with patients and families for understanding of diagnosis, prognosis, and plan of care

  • Determine effectiveness of treatment plan and documents patient care outcomes

  • Revise and modify the treatment plan to achieve patient goals

  • Present hospice information effectively

  • Collaborate with ministry Clinical Social Work Departments to assure continuity of psychosocial support for hospital patients and families

  • Collaborate with hospice, facility and home care MSWs to assure continuity of psychosocial support for patients and families discharged to home or facility, on home care or hospice

  • Understand the psychosocial needs of frail patients with advanced illness

  • Meet required documentation standards

  • Assist program manager in the development of assessment tools and program-specific policies and procedures

  • Respond in a timely manner to meet physician and patient needs

  • Promote palliative and hospice care to patients, families, community and other health care professionals:

  • Serve as a palliative psychosocial resource to hospital staff and team members

  • Develop staff and physician relationships that facilitate ongoing palliative care referrals

Required qualifications for this position includes:

  • Master's Degree in Social Work (MSW)

  • Current California Licensed Clinical Social Worker (LCSW)

  • Three (3) years of Inpatient, outpatient, or home-based social work

  • Demonstrated experience in counseling and group work

  • Experience working with the legal issues arising from illness and patient care issues

  • Excellent verbal and written communication skills.

  • Working knowledge of computer applications, i.e. Microsoft Office

  • Maintain and improves skill level through participation in continuing education

  • Regularly attends and actively participates in staff meetings

  • Complete all initial and annual required learning relevant to licensure and this role

  • Comply with all relevant laws, regulations, and policies

  • Contribute to a safe work environment

  • Demonstrated evidence of experience in a variety of care settings, such as palliative, hospice, clinic, hospital, home, i.e., ability to:

  • Work independently as well as collaboratively on a small interdisciplinary team,

  • Provide and model interdisciplinary palliative assessment and management of physical, psychosocial and spiritual symptoms during aggressive treatment and end of life care

Preferred qualifications for this position includes:

  • Advanced training in palliative care, geriatric or medical social work

  • Spanish speaking
